What forms of payment does Equilibrium Nutrition, PLLC take?
Credit Cards (including: American Express, Discover, Visa, and Mastercard. Debit cards, Health Spending Accounts (HSA) cards, and Flexible Spending Accounts (FSA) cards) are accepted.
Will my health insurance cover dietitian services?
Erica is a BCBS of IL in-network provider in some PPO and Blue Choice PPO plans. Clients are encouraged to confirm coverage benefit and any out of pockets expenses (e.g. copay and/or payment towards deductible).

Summary of questions to ask to verify your nutrition benefits

Do I have coverage for nutrition counseling (aka Medical Nutrition Therapy) as a preventative benefit or do I need a diagnosis? If a code is requested:, offer: 97802 & 97803 first (if not accepted, indicate 99404)

Do I need a referral to see a Registered Dietitian?

Are my visits covered as prevention (code z71.3) or are any diagnoses covered on my particular insurance plan?

Is there an associated cost for me if I choose to have the appointment as a telehealth visit versus in person visit? Note: Erica only offers telehealth visits.

Note: Erica will wait for the claim to be processed to determine whether or not you have a co-pay and then charge the credit card you have on file for the co-pay amount.

Otherwise, Erica’s consulting services are fee for service. She is happy to provide a Super Bill upon request that can be used to self submit to your insurance company.

Initial Appointments: 55-60 minutes
Follow Up Appointments: ~45 minutes*
* Bi-weekly follow up visits for a minimum of 2-4 months are recommended to support effective, sustainable lifestyle change .
